Documents Required for Hospital Treatment at Indrayani Hospital
We wish to provide you with the most efficient healthcare, but we require some basic documents before we can start your hospital treatment.
- Aadhar card/PAN card
- Evidence of Cancer Biopsy Report (Tissue Diagnosis) (If available)
- Cytology Test results (If available)
- Imaging as applicable (If available)
For patients coming through the MJPJAY and PMJAY program, please bring the following documents:
- Ration Card (Yellow/Orange)
- Aadhar Card
- Letter from Tehsildar/Rationing Office/Parimandal Adhikari (IF REQUIRED)
(The name on the report and the ration card must match)
