We wish to provide you with the most efficient healthcare but we require some basic documents before we can start your treatment.
- Aadhar card/PAN card
- Evidence of Cancer Biopsy Report (Tissue Diagnosis) (If available)
- Cytology Test results (If available)
- Imaging as applicable (If available)
For patients coming through the MJPJAY and PMJAY program, please bring the following documents:
- Ration Card (Yellow/Orange)
- Aadhar Card
- Letter from Tehsildar/Rationing Office/Parimandal Adhikari (IF REQUIRED)
(The name on the report and the ration card must match)
